As a tax filer, the documents you need to send depend on your situation, such as whether you’re an individual, self-employed, or a business owner.
Here are the common documents you may need:
- Online Application – REQUIRED
- Valid Identification – REQUIRED
- Drivers License
- State ID
- Government ID
- Passport
- Proof of Dependents – can provide any of the following, however, if they do not have, the fee we will conduct due diligence, and the rate will increase
- Birth certificate
- Medical Records (that show address, dependents name & taxpayers name)
- Lease agreement (that shows address, dependents name & taxpayers name)
- Supporting documents that prove this is your dependent, the dependent relationship to you and you provided support in the home

Income Forms examples below:
- W2 – if taxpayer was an employee – REQUIRED, If applicable
- 1099 NEC – if taxpayer was an independent contractor NOT REQUIRED if they have total & name of the company
- 1099 R – if taxpayer pulled money out of their 401k – REQUIRED, If applicable
- 1099 MISC – if the taxpayer received rent, royalties, commission, etc… NOT REQUIRED, if they have the total & name of the company

Business Owners (NOT REQUIRED, DUE DILIGENCE REQUIRED by TEAM) can provide any of the following, however, if they do not have, the fee we will conduct a due diligence and the rate will increase
- Income information
- Expense information
- Profit & Loss
- Records that show what you made & what you paid (if a business owner)
- Any business records
- Business licenses
- Business articles
- Any and all records to support business filing
